CT CORONARY ANGIOGRAM

LEFT ANTERIOR DESCENDING STENOSIS IN 27 YEAR OLD MALE

FINDINGS:

  • Left dominant coronary circulation.
  • Total calcium score = 0 Agatston units (area-density method)
  • Atherosclerotic plaque involving the ostio-proximal LAD for a length of 19-20 mm, causing near complete (99 %) occlusion at the LAD ostium and severe (70-80 %) stenosis of the rest of the proximal LAD
  • Narrow caliber mid LAD showing myocardial bridging for a length of 22-23 mm and maximum depth of 7.5 mm; with the mid LAD seen coursing to the right side of anterior interventricular septum.
  • The distal LAD shows poor contrast opacification.
  • D2, S1 & S2 are not well opacified.

Discussion:

Coronary heart disease, or coronary artery disease (CAD), is characterized by inflammation and the buildup of and fatty deposits along the innermost layer of the coronary arteries. 

Symptoms of coronary artery disease include:

  • Heaviness, tightness, pressure, or pain in the chest behind the breastbone
  • Pain spreading to the arms, shoulders, jaw, neck, or back
  • Shortness of breath
  • Weakness and fatigue.

Most common risk factor in young age:

1: Smoking.

2: Hyperlipidaemia.

3: Positive family history of CAD. 4: Physical inactivity.
